Add extraction and hoisting of a leading agent visibility note so Markdown pages can place the agent note before the page title. Introduce extractLeadingAgentVisibilityMarkdown in visibility.ts to detect and remove a leading AGENT_VISIBILITY_MARKDOWN block and return the remaining markdown. Use this in getPageMarkdownText to prefer the extracted agent note (falling back to injectedAgentNote) and adjust the joined markdown order. Add tests in visibility.test.ts to cover extraction behavior.
